Matching Service Application Form for Juvenile Justice System Community Service Hours---"Request for Help" in Finding a Nonprofit Match

On this page you will find the Matching Service for Juvenile Justice System Community Service Hours which is a Free service. Simply fill out the “Request for Help” form below and you will receive assistance in finding a philanthropic organization where you can do your Community Service Hours.
Important! Please have your parent use their contact information and manage all internet correspondence. Please use the same caution that you would anywhere else on the internet. Your request for a volunteer match is being reviewed by the staff & volunteers at other nonprofit organizations, not CommunityCause.org volunteers.
  • Please share your first name only. For security purposes it is best to never give your last name over the internet.

  • What is your age? Some nonprofits have age limits for youth they will allow to volunteer because of requirements from their insurance company.

  • Due to the requirements of the type of volunteer position to fill, some nonprofit organizations may want to know.

  • What city do you live in? If you live in a large city, you may want to put north, south, east, west as an indicator. For example, North Phoenix or South Scottsdale.

This feature of the CommunityCause.org website strives to help you find a nonprofit organization that is located near where you live.

If you are willing to travel some distance in order to fulfill your Community Service hours, you may want to include how many miles away from your home that you are willing to travel.

  • What are the major cross-streets nearest to your home? Major streets only please. No exact address. We are looking for the larger streets that most people would know.

This feature of the CommunityCause.org website strives to help you find a nonprofit organization that is located near where you live. By typing in your major cross streets you can help nonprofit organizations to pinpoint whether they are a good volunteer match for you.

  • How many Community Service hours do you need to do? How many volunteer hours are required in order for you to be compliant with the Juvenile Justice System?

It is important to answer this question because it will save time. Some nonprofit organizations have a minimum number of hours that they require in order to accept a volunteer.

  • Please list which days you are available to volunteer.

Please note the hours you prefer to do your Community Service hours.

The more specific you are in answering this question, the easier it will be for a nonprofit organization to know if they are a match.

  • When would you ideally like to begin doing your Community Service hours? If your answer is, "As Soon As Possible--A.S.A.P.", then please type in today's date.

This is important information for nonprofit organizations to have because sometimes they have special events at different times of the year and they need to ramp up the number of volunteers helping.

  • When do you need to have your Community Service hours completed by? What is the Compliance Date that the Juvenile Justice System has set? What is the deadline?

This helps the nonprofit organization to better plan. It also helps them to know if they will be able to offer you enough opportunities to complete your required number of Community Service hours by the compliance date deadline.

By filling in this information it will help nonprofit organizations to know if they will be a good volunteer match for you.

  • What is the name of the Teen Court representative or the Juvenile Justice System Probation Officer to whom you must report your Community Service hours. Who is your contact person?

  • What legal charges do you have that require you to do Community Service hours? What is the actual name of the charge(s)? (Ex: shoplifting, curfew violation, assault, criminal damage, etc.)

If you wish, you may type in any other information that you think a nonprofit organization may need to know about why or how you got the particular legal charge.

This is important information to include because, for legal or insurance reasons, nonprofit organizations may not be able to accept Community Service hours for all charges. By filling in this section, you will be saving everyone a lot of time and helping to make an accurate volunteer match.

  • For legal & insurance purposes some nonprofit organizations are only able to accept Youth Volunteers who can do their Community Service hours while accompanied by a parent or other responsible adult.

Please Note: If you put "Yes" you will vastly increase the number of volunteer opportunities available to help you complete your required Community Service hours.

If you select, "Yes, but I prefer to volunteer alone", that will leave you open to accept volunteer opportunities that either do or do not require that a parent or other responsible adult be present.

If you select "No", there are still opportunities to volunteer and complete your Community Service hours. Due to legal & insurance restrictions some nonprofit organizations will not be able to accept you as a volunteer.

If you wish, you may even select the option, "Yes and I prefer an opportunity to volunteer along with my parent or other responsible adult." Nonprofit organizations who can offer that option to volunteer will contact you. Please select the answer that best reflects your situation.

    • Thanks for Sharing Your Time, Talents & Skills to Make a Positive Difference in Your Community!

Are there any skills that you have that you would especially like to use? Are you a musician, artist, math whiz, computer expert, photographer, videographer, social media lover? Do you want to do physical work? Would you like to volunteer from home? Are there any special accommodations that you would like to request from the nonprofit organization?

Please also include information about what type of volunteer work interests you. Are you especially good with animals or specific types of people? Do you want to help Animals, Seniors, Veterans, Children & Teens, Social Justice Issues, the Homeless, the Environment, etc.? If you have "No Preference", please note that.
